1) All requests must be submitted in writing, preferably in electronic format (Word or PDF) to amybrf@yahoo.com. Supporting documentation (brochures, annual reports, etc.) may be scanned and attached to your email, or sent as a separate package through the U.S. Mail.
*If submitting requests via U.S. Mail, please forward eight (8) collated sets to the address listed below.

2) Identify the organization making the request and show evidence of 501(c) 3 status.

3) Identify other sources of funding for this project.

4) Provide the organization’s mission statement and annual financial statement.

5) State amount requested and give detailed project description. Our preference is to provide a definitive amount of funding for a specific expenditure relating to education, health or cultural needs of a local organization.
